DECLARATION

of informed consent for participation in a conference and processing of personal data

I HEREBY DECLARE THAT:
  1. I agree and give my consent to Burgas Free University (BFU) to process my e-mail and a contact phone number.
  2. I give my consent to BFU to contact me through its employees by the telephone number and / or e-mail provided by me in connection with my participation in a conference of BFU.
I ACKNOWLEDGE THAT:
  1. My personal data will be used by BFU on the territory of the Republic of Bulgaria, will not be provided to third parties and will serve only for the purposes of my participation in the conference of BFU.
  2. I am aware that my personal data will be stored in paper and electronic formats according to the legal data protection requirements.
  3. I am aware that the retention period of personal data collected by BFU in connection with my participation in the conference coincides with the deadline for completing all the activities related to the conference and my participation in the conference.
  4. I am aware that I am entitled to receive information regarding the stored personal data at any time; I have the right to access my personal data and the right to transfer the data to another administrator.
  5. I am aware of the fact that I may request from BFU to correct my personal data, delete it, and restrict its processing.
  6. I am aware that with respect to deletion, I have the right to request from BSU to delete the personal data provided with my consent without delay, and the administrator has the obligation to delete my personal data without delay.
  7. I am aware that the personal data will be processed by BFU and the contact person appointed by BFU is Prof. Dr. Milen Baltov, e-mail: mbaltov@bfu.bg, phone: +359 56 900 565.
  8. I am aware that I have the right to withdraw my consent to the processing of personal data, partially or fully, at any time, for which I shall notify the person designated under paragraph 7 in writing at the following address: Milen Baltov, office 309, Burgas Free University, 62, San Stefano Blvd. Burgas, 8001.
  9. I am aware that I am entitled to lodge objections and complaints regarding the processing and storage of personal data to the Commission for Personal Data Protection, which is the supervisory authority in the Republic of Bulgaria.

I declare that I give my consent to the processing of personal data freely, of my free will, and I guarantee that the information provided is accurate.
